Please pick up any prescribed medications and use them as directed. If you have any difficulty obtaining the medication or have questions, please contact us immediately.
If labs or radiology testing has been ordered, please have it completed ASAP.

We accept most commercial insurance plans and Medicare. Please contact our office to verify acceptance of your plan. Please be familiar with your deductible and coinsurance. These are the largest contributors to out-of-pocket costs, even when insurance is accepted.
We accept credit cash, check, credit cards and CareCredit.
