For a flat rate, we offer a comprehensive option that includes:
– An evaluation of your pertinent medical notes, laboratory and pathology results
– Pertinent medical exam
– Comprehensive plan
Before your visit
Once you have an appointment booked, electronic intake forms will be sent over. Please complete the electronic intake forms. Nobody likes to fill out forms. But this consolidated information helps us to understand your unique medical history, anticipate issues and suggest tailored treatment options.
Please take a photo of any prescription or over-the-counter topicals (creams, shampoos, washes, etc…) currently or previously used. Or just bring them in.
At least one day prior to your appointment, we will need:
– Completed intake forms
– Valid ID
– Insurance cards (if applicable)
* If we do not have all these items in time, we may have to reschedule your appointment.
Please plan on arriving 15 minutes prior to your scheduled appointment time.
If you cannot keep your appointment or will be delayed, please call us as soon as possible.
After your visit
Please pick up any prescribed medications and use them as directed. If you have any difficulty obtaining the medication or have questions, please contact us immediately.
If labs or radiology testing has been ordered, please have it completed ASAP.
If you have any questions or concerns, please do not hesitate to contact us.
If a follow-up appointment was recommended, please keep it. We really want to know how you are doing.

We accept most commercial insurance plans and Medicare. Please contact our office to verify acceptance of your plan. Please be familiar with your deductible and coinsurance. These are the largest contributors to out-of-pocket costs, even when insurance is accepted.
We accept credit cash, check, credit cards and CareCredit.
