The treatments can be very different, which is why getting the label right matters.

Q: What makes ordinary acne different?

A: Acne often includes a mixture of blackheads, whiteheads, inflamed pimples, and sometimes deeper nodules or cysts. The face, chest, shoulders, and back are common sites. The presence of comedones—blackheads and whiteheads—is an especially useful clue.

Q: How can rosacea look like acne?

A: Rosacea can cause acne-like bumps on the central face, often along with flushing, persistent color change, burning, stinging, or sensitivity. On darker skin tones, redness may be less obvious and warmth, sensitivity, brown-violet color change, or bumps may be more noticeable.

Q: What is folliculitis?

A: Folliculitis is inflammation of hair follicles and can be caused by bacteria, yeast, friction, occlusion, shaving, or other triggers. It may appear as many similar-looking small bumps or pustules and can itch or feel tender.

Q: What about tiny bumps around the mouth?

A: Perioral—or more broadly peri-orificial—dermatitis can resemble acne around the mouth, nose, or eyes. It may be dry, flaky, itchy, or burning. Some topical products, including inappropriate use of topical steroids on the face, can complicate the picture.

Q: Can hidradenitis look like acne?

A: Yes, especially early on. Recurrent deep painful bumps in the armpits, groin, under the breasts, or other skin folds may be hidradenitis suppurativa rather than ordinary acne. Repeated lesions in the same areas, drainage, tunnels under the skin, or scarring are important clues.

Acne Lookalikes at a Glance

Blackheads/whiteheads + mixed pimples → acne is more likely

Central facial bumps + flushing/sensitivity → consider rosacea

Uniform itchy or tender follicle-based bumps → consider folliculitis

Small mouth/nose/eye-area bumps + dryness/burning → consider perioral dermatitis

Recurrent deep painful bumps in folds → consider hidradenitis suppurativa

When should you stop experimenting at home?

If a “breakout” is painful, scarring, recurrent, suddenly worsening, or simply not responding to reasonable acne care, it is worth getting a diagnosis. Throwing more benzoyl peroxide, acids, scrubs, and spot treatments at the wrong condition can make irritated skin even angrier.

Learn more about acne care at Borealis Dermatology.

Borealis Dermatology sees patients in Garden City, Syosset, and Bellerose, NY. Visit our locations page or Book Appointment.

Continue reading about why facial redness is not always rosacea.

Related care: Acne treatment · Rosacea and facial redness care · Hidradenitis suppurativa care

Trusted Sources & Related Reading

American Academy of Dermatology: Is that really acne?

American Academy of Dermatology: Perioral dermatitis

American Academy of Dermatology: Folliculitis

Some bumps that look like acne are actually rosacea, folliculitis, or perioral dermatitis.
