Before your visit
First, thank you for considering us for your dermatologic needs.
Please complete the electronic intake forms. Nobody likes to fill out forms. But this consolidated information helps us to understand your unique medical history, make a diagnosis, and suggest treatment options.
Please take a photo of any prescription topicals (creams, ointments, washes, etc…) you currently or previously used. Or just bring them in.
Please submit the intake forms, ID and insurance cards at least one day prior to your appointment.
Please plan on arriving 15 minutes prior to your scheduled appointment time.
If for any reason you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible.
After the appointment
Please pick up your medications. If you have any difficulty obtaining the medication or have questions, please contact us immediately.

Insurances: We accept most commercial insurance plans and Medicare. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.
We accept credit cards, checks, and cash. We also offer a flexible payment plan. Please see our Patient Coordinator for details.
