Repeated intense itching and scratching can produce firm nodules, disrupt sleep, and leave scars or dark marks.

Care focuses on breaking the itch-scratch cycle and treating the inflammation driving it, with therapy scaled to how widespread and disruptive the disease has become.

What does prurigo nodularis look like?

Firm bumps may be scratched, crusted or thickened, often on the arms and legs. They can be pink, brown, purple or darker than nearby skin. PN involves inflammatory and nerve signaling; telling someone to “just stop scratching” is not a treatment plan.

Confirming the diagnosis

We examine the bumps and look for other contributors to itch. Selected patients need a biopsy or blood tests. Eczema may coexist, but PN requires its own assessment.

Prurigo nodularis treatments and medications

Skin-directed treatment: Prescription topical corticosteroids, selected nonsteroidal treatments, protective dressings or injections into individual nodules may reduce inflammation.

Targeted medicines: Dupixent (dupilumab) and Nemluvio (nemolizumab-ilto) are FDA-approved for adults with PN. Dupixent targets IL-4/IL-13 signaling; Nemluvio targets the IL-31 receptor pathway involved in itch.

Phototherapy: Narrowband UVB may help selected patients. Whole-body NB-UVB is available at Syosset and Bellerose; see our phototherapy options.

Other approaches: Off-label medicines may be considered when appropriate, with individualized monitoring.

These therapies have different risks and practical requirements. Dupixent can cause eye symptoms and allergic reactions. Nemluvio can cause hypersensitivity, headache or eczematous rashes; vaccination and pregnancy plans should be reviewed. Official medication links include full safety information. Neither drug is a general treatment for every unexplained itch.

What improvement can look like

Itch and sleep may improve before the nodules flatten. Residual pigment changes can persist after inflammation settles, especially in darker skin. Follow-up assesses both symptoms and healing rather than color alone.

Common questions

Is PN contagious?

No. It cannot be passed to another person through contact.

Is every itchy bump PN?

No. Several conditions can look similar. Our chronic itching evaluation helps identify the underlying cause.

When should I call?

Call for worsening pain, warmth, pus or fever, which can signal infection.

Further reading: AAD guidance on prurigo nodularis.

Schedule an evaluation

Appointments are available in Garden City, Syosset, and Bellerose, NY. Book Appointment or call (516) 246-8800.

Clinical example; appearance varies between patients. Photo: Masryyy, CC BY 4.0.
